After scheduling an appointment with Beulah Family Dentistry, you will receive a link to the required patient forms via text or email to complete digitally right from your phone, computer, or tablet.
Forms can also be completed in our office; please arrive 15-20 minutes prior to your appointment time to ensure enough time for completion and bring your ID and insurance card.
Required forms include:
- New Patient Form
- Health History
- Dental History
- Dental Insurance Form (if applicable)
- Office Policy
- Release of Records Authorization
